Root cause: In the two-group layout, !fs|ref|… routes to group 2 and the inode key to group 1, so lease refresh uses 2PC. startTS is the caller's read snapshot. That means two concurrent refreshes of the same handle can carry the same FSM transaction identity (primaryKey, startTS). The FSM treats the second PREPARE as an idempotent replay and overwrites the first transaction's intent. The later COMMIT then finds a commit record with a different commit timestamp and returns ErrTxnInvalidMeta, which surfaces as ErrTransactionOutcomeUnknown and then EIO. In one logged case the recorded commitTS is larger than the requested one, which shows these are two different transactions, not a retry of one.
Fix:ShardedCoordinator now reserves the identity in-process for the whole 2PC. A colliding transaction is rejected with a definitive ErrWriteConflict before it proposes anything. The filesystem and adapters already retry on that error, at a fresh snapshot.
Behavior change / risk: A second cross-shard transaction on the same primary key with the same snapshot, dispatched while the first is in flight on the same node, now gets a write conflict. Before, it silently overwrote the first transaction's intent. The guard is process-local. Writers of the same key on different nodes with the same snapshot are still not covered. Closing that gap needs the commitTS in the lock identity, which is the existing prepare-evidence path, and that is a separate, design-doc-level change.
2. Update review approval label (13:21) — fixed
A single GitHub API 503 on GET /pulls/1380 failed the run because actions/github-script uses retries: 0. I set it to retries: 3.
Not fixed (analysis only)
filesystem-rename (06:05): the result was :valid? :unknown, from a single final read on n1 that got EIO. The cause was FUSE open failed ... transaction outcome unknown at 06:38:30. There was no nemesis and no leader change. This is a single-group (one-phase) cluster, so the collision above cannot cause it. The original Publish error is discarded by publishOpenOperationAttempt → resolveOpenAttempt, so the cause is not visible in the logs.
filesystem-node-crash (12:04): truncated.dat did not recover on fs-n3 after FUSE release uncertain: transaction outcome unknown around the node crash. The cause was not identified.
Test on renovate/go-1.x: TestMigrationQuorumCommitFollowsForcedLeadershipTransfer failed with transaction outcome unknown: context deadline exceeded under leader churn with -race. This is unrelated to this change.
Test evidence
New regression test TestShardedCoordinatorDispatchTxn_RejectsConcurrentSameIdentity. It failed before the fix (the second transaction prepared and committed under the in-flight identity) and passes after it (-race -count=3).
Local reproduction: --raftGroups 1,2 --shardRanges ':!fs|op|=1,!fs|op|:=2', legacy mode, scripts/benchmark-fuse.py --layout two-group. Before the fix, each run logged about 200 commit_ts mismatch lease-refresh failures. After the fix, 3 of 3 runs logged 0.
go test -race ./kv/... ./internal/filesystem/... passes.
golangci-lint v2.15.0 run ./kv/...: 0 issues.
Jepsen: not run locally beyond the FUSE benchmark above.
Self-review
Data loss: The guard only rejects before any proposal, so it cannot lose a committed write. It also prevents the second PREPARE from overwriting the first transaction's intent.
Concurrency: The map is under a mutex. The reservation is released by defer on every return path, including outcome-unknown. A lock resolver never dispatches under the same identity.
Performance: One map insert and one delete per cross-shard transaction under a short mutex. One-phase transactions are unaffected.
Consistency: The loser gets a definitive OCC write conflict, which is correct for two writers of one key from one snapshot.
Coverage: Regression test added. The multi-node gap is noted above.
startTS is the caller's read snapshot, so two independent cross-shard
transactions on the same primary key can carry the same FSM identity
(primaryKey, startTS) -- e.g. two concurrent FUSE lease refreshes of one
open handle in the two-group layout. The FSM treats the second PREPARE as
an idempotent replay and overwrites the first intent; the later COMMIT then
fails with "commit_ts mismatch ... txn meta invalid", which surfaces as
ErrTransactionOutcomeUnknown (EIO through FUSE). This is the cause of the
filesystem-benchmark-multigroup-legacy failures in the scheduled Jepsen run.
Reserve the identity in the coordinator for the duration of the 2PC and
reject a colliding transaction with a definitive write conflict before it
proposes anything, so the caller retries at a fresh snapshot.

The coordinator reservation reduces same-instance transaction collisions. The remaining introduced concern is a test that can stall until timeout if dispatch fails early; add a bounded wait. Cross-node collision protection remains an existing limitation.

On CodeRabbit's follow-up about kv/sharded_coordinator.go:563-567 (a PREPARE from a different coordinator with the same (primaryKey, startTS)): agreed, and this PR deliberately leaves it as is.
The in-process reservation covers the collision the CI failures came from (two concurrent lease refreshes of one handle on the same node; the ref key embeds the client ID, so only one node writes it). As the review notes, the FSM can't reject an existing identity outright because a legitimate PREPARE replay must succeed (kv/shard_store_prepare_evidence_test.go). Distinguishing a replay from a different transaction across nodes means carrying the commit timestamp in the lock identity on every group, which is what the prepare-evidence path does when enabled. That's a lock-format change that needs its own design doc, so it isn't in this PR. The PR description's "Behavior change / risk" section already records the gap.

test (ubuntu-latest) failed on e66621f in TestMigrationQuorumPartitionAcceptance (adapter/migration_quorum_partition_acceptance_test.go:720): large-write/snapshot phase must finish within its own budget — context deadline exceeded after 1m2.5s against a 60s phase budget.
I don't think this is this PR's:
The test drives a single-group quorum cluster through TransactionManager directly and never reaches ShardedCoordinator.dispatchMultiShardTxn, the only code path this PR changes.
The phase's work actually completed (the ACKed write index 23 and native snapshots on all four members were logged); the overrun happened while closing the nodes after the 64 MiB snapshot, i.e. a wall-clock budget on a busy -race runner.
A sibling in the same suite (TestMigrationQuorumCommitFollowsForcedLeadershipTransfer) failed the same way yesterday on renovate/go-1.x, before this PR existed.
I've re-run the failed job once to confirm. There's no fix for the budget flakiness yet; I'm not widening this PR with one.
