Why

Failed release runs can leave a version tag behind while its published GitHub release is missing. Later green runs can hide that gap.

What

Add an explicitly enabled, read-only diagnosis that checks the original run, source and publication, and refuses incomplete or conflicting evidence. It helps operators identify the gap while granting no publication authority.

Deferred: #587 — publishing missing releases, repairing channel notes and protected consumer recovery.

Fixes #598
Part of #587

User evaluation at 5b7fe065704bfe15ebae84c8bb919656ecd6fef4: built the actual inspector and ran it read-only against Platform's two public incidents. Original runs 38418390528/1 and 38573634755/1, their immutable producers, exact sources and tags v1.222.1 / v1.223.3 were independently read twice. Both returned missing-github-release, with publication_scope=published-release-by-tag, authorization=none, publication_success=false, and explicit exclusions for draft inventory and channel metadata. Nothing was published or repaired.

The native HTTP suite passes with race detection and 94.2% statement coverage. It exercises the actual client, only GET requests, identity/configuration conflicts, redirects, bad status/media/JSON, duplicate fields, truncated/oversized reads, timeouts and changed second observations. Authoritative tree modes reject symlinks even when Contents metadata calls them files; decoded configuration bytes must match their Git blob identity. The workflow boundary suite executes the shipped admission and anonymous immutable-source download and rejects changes to credential scope/order, source identity, default-off routing and required CI coverage.

All fourteen repository baseline checks, Go vet, shell checking, README interface parity, CI selection and catalogue credential-graph checks pass locally. Zizmor reports no findings on the new workflow. The host's actionlint 1.7.12 does not recognize the documented job.workflow_repository / job.workflow_sha properties already used by the catalogue; GitHub's context reference confirms these properties bind the called workflow source. No lint rules were changed.

Self-review covered correctness, source/credential confinement and simplification. The symlink proof and unpublished-record evidence labels were corrected before this review request. This PR completes diagnosis child #598 only; #587 retains publication, channel notes, legacy compatibility, consumer recovery and rollout/removal of the temporary recovery path.

📝 Walkthrough

The change adds a read-only Go inspector for release incident evidence and a default-off GitHub Actions workflow that can run it manually or as a reusable workflow. The inspector validates incident inputs, checks repository, run, tag, source configuration, and release data, then repeats its reads before reporting whether a matching published release exists. Tests cover the inspector and workflow constraints. CI runs those tests, and the README and AGENTS.md document the workflow and its diagnosis limits.

Priority: ➖ Normal

Severity of issue fixed: Medium

Merge Risk: 🟡 Moderate · up to 5b7fe

The new diagnosis fails for repositories whose GitHub release already exists instead of reporting that the release is present. CI coverage for the new workflow also does not meet the repository's required-check contract. Fix both before merging.

Resolution record for review 5536034142 at repaired head 7bb142590928f2529e823eeb7ec3929f3985bae2:

  • Both inline findings are fixed: branch-valued publication metadata is supported while tag/source verification remains mandatory, and a credential-free direct disabled-mode caller now gates CI with complete inventory/aggregation coverage. Native regressions and the actual inline parity guard pass.
  • The docstring-coverage warning is addressed with purpose and safety-contract comments for the new command's helpers and native controls, rather than suppression.
  • The hard-coded-token warning names GH_TOKEN=must-not-forward, a deliberately synthetic fixture sentinel. The executed curl fixture verifies this value is absent from the retrieval environment. It is not a credential and grants no access.
  • The SHA-1 warnings in production and fixture code refer to Git's required blob-object format. This consistency check compares bytes with the GitHub tree's existing object ID; it is not a signature, password hash or independent trust decision. Authentication and source evidence come from the fixed-origin GitHub API. Substituting SHA-256 would cease to compare Git object identities. The code now makes that boundary explicit; no rule was suppressed.
  • LanguageTool's capitalization suggestions occur inside literal .github/... paths and links. Their case is intentional; changing it would name different paths.

All reported body findings are fixed or specifically refuted above: body_findings=0-resolved@7bb142590928f2529e823eeb7ec3929f3985bae2. This does not substitute for a fresh substantive review and hosted checks at this head.

Resolution record for review 5536034142 at repaired head c9eda654a3396d46247e1a6c5b0a4b1a40cf071e:

All reported body findings are fixed or specifically refuted above: body_findings=0-resolved@c9eda654a3396d46247e1a6c5b0a4b1a40cf071e. This does not substitute for a fresh substantive review and hosted checks at this head.

cr:no-gate@c9eda654a3396d46247e1a6c5b0a4b1a40cf071e — provider quota, not a review verdict. Freshly read this PR's authenticated limit record, updated 2026-10-15T16:07:33Z, which states the next included review is available in 42 minutes (16:49:33Z). Its command refusal is updated 16:07:35Z, with no later substantive review. Observation: 2026-10-15T16:19Z; scope: this public PR. The no-gate guard admits this head. All earlier findings have specific fixes or assessments; a new head does not reset the active service limit. Advancing to Codex under the current-head request lock.

cr:no-gate@0247eeac41fe236d9745fc97a6815393f5cf0c4c — provider quota, not a review verdict. At 2026-10-15 16:29:59 UTC, I freshly read this PR's authenticated limit record, updated 2026-10-15T16:07:33Z: the next included review is available in 42 minutes (16:49:33Z). Its command refusal confirms that this PR was not served. No later substantive CodeRabbit review supersedes it. The no-gate guard admits this head. Earlier findings have specific fixes or assessments; the new head does not reset this active PR-scoped limit. Advancing to Codex under the current-head request lock.

Ready at 0247eeac41fe236d9745fc97a6815393f5cf0c4c:

  • Native CI 38769302990 is successful at this exact head, including the release-contract HTTP/race tests and the disabled reusable-workflow interface with no credentials.
  • Fresh substantive Codex review names this head and reports no major issues. All five native review threads are resolved; body findings are fixed or specifically refuted. CodeRabbit's active included-review limit is a provider quota, not a review or merge satisfier.
  • Local baseline checks, native HTTP refusal controls, race tests and vet pass. The actual compiled command independently inspected both original Platform incidents twice and returned missing-github-release, authorization: none and publication_success: false for each.
  • Every readable required check and required workflow passes. The configured code-quality rule has no readable verdict and remains UNVERIFIED; the protected merge API must enforce it. No administrator bypass or protection change is requested.

After merge, I will verify the actual default-branch workflow in disabled mode and against both incidents with the repository-scoped read-only App credential. This PR diagnoses the missing records; #587 retains publication recovery and the protected consumer outcome.
