Add typed Decisions questions and answers, public type exports, and usage documentation. Preserve mixed question types and inline request objects.
A real mixed request returned noul, choice, and score answers with confidence, probability maps, the score legend, and token usage. Consumer checks cover the discriminated request and response types.
