Separate cached results by resolver.

If one resolver returns ResolverNotFound, this branch now caches null. A subsequent lookup for the same name and key on another chain or resolver can receive that cached null instead of its existing record for up to one minute. Include the effective chain and resolver address in the cache keys for both resolveText and resolveAvatar. (raw.githubusercontent.com)
