fix(api): preserve client identity and websocket introspection policy - #2128
Preserve the client IP through the built-in nginx Unix-socket proxy so OIDC request limits apply independently to each source, and enforce the disabled-sandbox introspection policy on WebSocket operations.
Why this exists
API 4.37.8 added OIDC rate-limit enforcement, but ordinary proxied clients shared the fallback tracker. One client could exhaust another client's sign-in allowance. Schema introspection also remained available through graphql-ws while the equivalent HTTP operation was rejected.
Resolution
nginx overwrites X-Forwarded-For with its observed client address. The existing rc.nginx modification now applies the identity correction to integrated API releases and retains the legacy Connect changes where needed. Static and dynamic patches agree.
The guard uses the actual TCP peer on TCP connections, including loopback. On the Unix socket, it accepts one valid proxy-provided address and rejects missing or ambiguous identity.
WebSocket operations reuse the existing parsed, selected-operation introspection check and read sandbox state for every operation before execution.
Reviewer considerations
Unix-socket access remains a local trust boundary. Direct TCP callers cannot choose their throttle identity with forwarding headers. Clients behind one NAT address still share that address's allowance. The nginx generator uses a quoted heredoc; its inserted variable must remain unescaped. The current state-cap and GraphQL resource-limit policies are preserved.
Verification
44 focused tests pass, including real Unix-socket requests, TCP header spoofing, live WebSocket sandbox toggles, selected operations, fragments, and blocked resolver side effects.
Execute the bundled nginx generator and verify exactly one unescaped client-IP directive; exercise saved-patch upgrade, repeated apply, and rollback.
API type checking, lint (including authorization lint), and production build pass.
Independent Architecture, Security, Tests/validation, and Checklist reviews completed; the one generator-escaping finding is resolved.
Installed PR artifact validation on a managed Unraid QA VM remains pending. This description will be updated with exact build and appliance results before the retest handoff.
Release gate and work intent
Work Intent: OS-982, OS-900, and OS-1073, following their failed QA results and the requested combined remediation.
Hold shipping until the installed build passes developer validation and Larry approves his retest. No merge or release is requested by this PR.
Summary by CodeRabbit
Security
Rate limiting now identifies clients using their connection address, or a validated forwarded address when no connection address is available. Invalid or missing forwarded addresses are rejected.
GraphQL WebSocket requests now follow the sandbox setting for introspection access and enforce the query-size limit.
Bug Fixes
GraphQL proxy requests now forward the client address.
Remote-access detection now uses Connect configuration, and older nginx configurations without legacy settings can be updated without errors.
The pull request changes throttler client-address tracking, adds sandbox-aware introspection checks for GraphQL WebSocket operations, and updates nginx configuration patching for proxy client identity and Connect remote-access settings.

Address tracking and throttling tests api/src/unraid-api/auth/fastify-throttler.guard.ts, api/src/unraid-api/auth/fastify-throttler.guard.spec.ts
The guard uses the socket peer address when available. Otherwise, it accepts only a valid IP address in x-forwarded-for and returns a 400 error when no accepted address is available. Tests cover TCP peer limits, separate routes and peers, and Unix-socket forwarded-address cases.
The WebSocket options factory checks parsed operations when sandbox mode is disabled and returns INTROSPECTION_DISABLED for blocked introspection. GraphModule supplies the sandbox-state callback. Integration tests cover selection forms, multi-operation documents, sandbox changes, and alias limits.

The modifier adds $remote_addr forwarding to matching GraphQL proxy blocks and guards legacy Connect edits on MYSERVERS=. The remote-access check uses Connect plugin state and connect.json values. Tests cover generated configuration, repeated application, and rollback.

On installations with a differently cased forwarding directive, protected API requests may be rejected. The fix is localized; merge with that configuration risk understood or remove directives case-insensitively first.

Minor · Functional Correctness · Quick win
Remove forwarding directives regardless of header-name case.
If an existing GraphQL block uses proxy_set_header x-forwarded-for, this case-sensitive replacement leaves that directive in place and adds another one. Node combines duplicate header values, so the Unix-socket throttler rejects the resulting value instead of accepting the client address. Match X-Forwarded-For case-insensitively before inserting the new directive. (raw.githubusercontent.com)

## Summary
Enforce the existing disabled-sandbox introspection policy on GraphQL
WebSocket operations before resolver execution.
## Why this exists
HTTP requests already reject schema introspection when the sandbox is
disabled, but graphql-ws uses a separate execution path that bypassed
that policy. Aliases and fragments must receive the same treatment on
both transports.
## Resolution
Reuse the existing selected-operation AST policy in the WebSocket
admission hook, with the existing sandbox configuration read for every
operation. This keeps HTTP and WebSocket policy consistent, including
operations on an already-open connection.
## Reviewer considerations
Review operation selection, fragment traversal, the dynamic sandbox
callback, and the Nest/graphql-ws adapter contract. The change preserves
ordinary __typename requests and existing token, cost, depth, and alias
limits. The nginx and OIDC changes remain separate in draft PR #2128.
## Behavior changes
With sandbox disabled, WebSocket __schema and __type operations return
INTROSPECTION_DISABLED before resolvers execute. Enabling sandbox
permits introspection; disabling it applies to subsequent operations on
the same connection.
## Implementation summary
- Share the existing introspection predicate across HTTP and WebSocket
transports.
- Construct the existing graphql-ws options with a dynamic sandbox
callback.
- Exercise the real WebSocket transport, selected operations, aliases,
fragments, resolver side effects, sandbox toggles, and resource
rejection.
## Verification
- 28 focused Vitest tests passed in introspection-plugin.spec.ts and
request-protection.spec.ts.
- Full API type checking, lint including authorization lint, and
production build passed.
- Fresh independent Security, Checklist, and Tests reviews found no
issues; the independent Architecture review confirms PATCH.
- Exact-head Main CI and CodeQL passed for
`e0dcd9172c1c9df54e5eb290bb177eda59e09f41`. The dependency audit is
advisory and reports 32 findings; this PR changes no dependencies. The
advisory project-coverage check reports a 0.01 percentage-point decrease
(53.95% to 53.94%); patch coverage passes and all modified coverable
lines are covered.
- Installed the published PR artifact on managed Unraid 7.4.0-rc.2 and
verified the installed policy bundle against the CI package digest.
- Appliance HTTP and graphql-ws checks passed:
direct/aliased/named/inline metadata denial, selected-operation
handling, normal queries, anonymous protected read/write denial,
100/101-alias boundary, and token-limit rejection.
- Sandbox off/on/off passed through nginx over HTTP and one continuously
open WebSocket (`connected=1`, no disconnect before cleanup).

## Risk
The admission hook affects GraphQL WebSocket operations. Normal query,
introspection, configuration-change, and resource-limit cases passed
locally and on the installed appliance. Anonymous protected WebSocket
operations return a session-validation error with no data; this change
does not alter the authentication context. External review suggested
stronger error assertions as an optional test cleanup; no runtime
finding was reported. Larry retest and production-release approval
remain pending.
