Feat: Keep Claude Code on moca's sandbox tools without a project settings file - #1495
With Cortex in the path, a moca repo needs no .claude/settings.json and no .mcp.json. CLAUDE.md loads, and the model still cannot run local tools.
An example script plugin does the deny's job in sessions that carry mcp__sandbox__* tools:
request: it removes moca's 17 local tools from the list the model sees, and replaces the result of any local call that already ran with an error;
response: it renames a local tool_use to blocked__<name>, so Claude Code answers "No such tool available" and runs nothing.
Both halves are needed: Claude Code runs a tool the model calls even when Cortex hid it.
Three gaps in the script plugin showed up in end-to-end testing and are fixed here:
A fail_closed slot refuses while a script did not start. Before, a script that failed to start was skipped and local Bash ran. Now every request gets a 502, plugin.script-failed, script <name> did not start, until a reload in which it starts. A new scripts not started metric shows it.
Slot folders added by a reload are watched. Before, a slot added by a config edit loaded its scripts but missed later edits until a restart. reloader.WithExtraWatch now takes func(*config.Config) []string and re-arms after each accepted reload. The startup hash now covers the watched folders.
The default slot stays when another slot is named. Before, adding a named slot switched off ~/.cortex/plugins. It is now inserted just before the first named slot that is built, since a response reader must come before a response rewriter. With it after, a user's response-reading script made the config fail to load.
The script is an example in examples/script-plugins/. Nothing in Cortex installs or references it. The install steps are in docs/script-plugin.md under "Examples".
go vet + go test -race for core and cmd/cortex (local, full, lite profiles); gofmt -l clean; go mod tidy -diff clean.
pytest tests/: 118 passed, 15 of them new for the example; ruff clean.
Live, on a scratch Cortex built from this branch:
a slot added to the config loaded on reload, without a restart, with the default slot before it;
a broken edit to the script gave a 502 with nothing sent upstream;
restored, a streamed local Bash call came back as blocked__Bash, and the upstream saw only mcp__sandbox__bash;
after a restart, an event in the slot folder triggered no reload.
Earlier spike, with a real model and the real moca sandbox (Claude Code 2.1.309): CLAUDE.md followed, only sandbox tools used, local Bash/Read refused, six resumed turns.
Known costs and open items
Streams are held back for every session while the script is loaded, because it rewrites responses. Measured: first text at 15.7s against 12.9s.
A fail_closed slot with no script in it passes all traffic. A copy into the wrong folder, or without the exec bit, protects nothing. This is a setup mistake, not something the plugin guards against: the docs' install steps run chmod +x and end with checking scripts loaded 1 on /v1/pipeline.
A down script blocks all traffic through its slot, not only moca's.
Claude Code's own tools stay visible:Skill, SendMessage, the Cron tools.
No Cortex, no protection: Claude Code must reach the model through Cortex.
The moca half (sbx up skipping the settings file) is deferred to a moca PR.
Deferred from review
A script that did not start is retried only when the slot is rebuilt, by a reload or a restart. Retrying it between reloads means changing the slot's script list while requests read it.
A reload that adds a script folder leaves the folder out of the stored hash, so the next event there rebuilds the pipelines once. Rehashing at that point could mark a script edit made after the build as running.
A script saved between the startup build and the reloader's start is not reloaded. The startup hash reads the folders at start, not as the build saw them. This predates this PR.
The example reads only Anthropic JSON and single-data:-line SSE. Other framings pass unchanged; the docs say so.
Review suggestions not taken: document that notStarted refuses even a would-be shadow script; note that mocactl sbx doctor's agent-config check fails under this setup; scriptFolders still watches on_error: off slots; some commit subjects are over 72 characters.
The default slot goes before the named slots, so the mirror case is refused: a response-rewriting script in ~/.cortex/plugins next to a named slot that only reads responses. The reload is refused, and the next restart does not start. List the default slot after that slot. TestLocalChain_ADefaultSlotRewriterBeforeANamedReader pins both halves.
agentop's plugin pane opens the first script slot whichever row is picked, because it resolves rows by name. The install check uses /v1/pipeline instead.
The example learns the shared default and pending: session ids as sandbox sessions, so one moca request there affects every unattributed request.
An edit in a folder a reload adds, made after the build read it but before it is watched, is not reloaded (CodeRabbit). This is the reload-time form of the startup item above.
Added an example plugin that limits local tools in identified sandbox sessions, replacing results from local calls and preventing subsequent calls in JSON and streamed responses.
Script slots with fail_closed enabled now reject requests with a 502 error when a script fails to start. In observing mode, the failure is recorded and requests continue.
Improvements
Newly configured script folders are watched after accepted configuration changes.
Local installs add the default script slot before eligible named slots when no default slot is specified.
Documentation
Expanded script plugin guidance and added setup instructions and limitations for the sandbox example.

Walkthrough
This PR changes script startup failure handling, reloader folder watches, and default-slot placement. It adds a MOCA sandbox script example that filters requests and rewrites tool calls in JSON and event-stream responses, with tests and documentation.
Changes: Script slots and MOCA sandbox example
Layer / File(s)
Summary
Fail-closed handling for scripts that do not start core/plugins/script/plugin.go, core/plugins/script/plugin_test.go, docs/plugin-catalog.md, docs/script-plugin.md, docs/superpowers/plans/..., docs/superpowers/specs/...
The plugin tracks scripts that fail to start. In a fail-closed slot, it refuses requests with status 502 and code plugin.script-failed. Observe policy records a shadow invocation and continues. Tests cover failure, recovery, timeout, and fail-open behavior.
Watching folders from accepted configs cmd/cortex/main.go, core/reloader/reloader.go, core/reloader/reloader_test.go, docs/superpowers/plans/...
The reloader derives watched folders from the active config and adds watches after accepted reloads. Watched-folder fingerprints contribute to change detection.
For local installs, the default slot is inserted before the first script entry whose error policy is not off when no entry without a directory already exists. Tests cover insertion order and pipeline construction.
The example filters local tools and replaces results from local calls in sandbox sessions. It renames local tool calls in JSON and event-stream responses. Tests cover request and response handling, and the documentation describes setup and operational limits.

Keep unapplied config bytes out of the active hash.
If the config file changes after Reloader.New but before seedHash reads it, seedHash marks the new bytes as active while the old pipelines still run. This can happen while Start registers folder watches. When the queued file event reaches reloadOnce, its hash matches lastHash, so the edit is skipped until another change. Re-seed the folder fingerprint using the config bytes associated with the active pipelines, or reload when the on-disk config differs.

Parse complete SSE events before checking tool calls.
If a content_block_start event uses multiple data: fields, each field fails the independent JSON parse. A CR-only stream also bypasses the scan. Both are valid SSE formats, so _block_sse can return the local tool call unchanged and Claude Code can run it. Parse the event’s combined data and preserve its framing when rewriting it. Add cases for both formats. (html.spec.whatwg.org)

Seed the folder fingerprint from the folders the active pipeline was built with.
seedHash runs in Start after watchExtra and hashes the fingerprint of every watched folder at that point. The caller builds the initial pipelines before New, and New does not capture a folder fingerprint. A script edit made between the initial build and seedHash therefore enters the baseline but not the active pipeline. A later event with the same folder contents matches lastHash and is skipped. The edited script then stays unloaded until the folder contents change again or the process restarts.
Capture the fingerprint used during the initial build, or rebuild when watch setup finds folder contents that differ from that build-time fingerprint.

Detect edits made before a new folder is watched.
If a reload adds a script folder, the builder can read its script before watchExtra adds the watch. An edit during that interval produces no folder event. The accepted pipeline then keeps the earlier script until another change triggers a reload. Reconcile the folder contents against what the builder loaded before reporting success, and trigger another reload if they differ.

Summary
The Cortex half is sound. A fail_closed slot now refuses while a script did not start (shadow under observe, with a metric). The reloader re-arms its folder watches on the goroutine that owns the watcher and seeds the startup hash again. addScriptSlot puts the default slot before the first built named slot, and a test pins the mirror case. The remaining reload gaps are listed under Deferred and are not blockers.
How it fits with mocactl sbx up (checked against moca upstream/main bbeca5f4):
It fails open where the settings deny does not. The script only acts when the request carries mcp__sandbox__*. If mocactl sbx mcp does not start (not logged in, sandbox gone, binary not on PATH), no sandbox tools are sent, the session is left alone, and a repo with no settings file has every local tool enabled. Today's deny holds whether the MCP server is up or not. See the inline comment.
--no-agent-config skips more than the settings file:.mcp.json, opencode.json, the CLAUDE.md/AGENTS.md block and the .sbx/agent-config.json record (sync.ts). So sbx rm will not undo the claude mcp add -s local, and sbx doctor's agent-config check fails on this setup.
rossoctl/moca#568 is closed: moca#572 moved moca's steering into the MCP server's instructions. What this still fixes is the project's own CLAUDE.md not loading.
LOCAL matches CLAUDE_DENY today (17 tools). Nothing catches drift until the deferred moca-side check exists.

suggestion: This keys protection on the sandbox MCP server being up. If mocactl sbx mcp fails to start (expired login, removed sandbox, mocactl not on PATH), Claude Code sends no mcp__sandbox__* tools, _is_sandbox returns False for a new session, and in a repo with no .claude/settings.json every local tool is live. Today's settings deny does not have this failure mode. At minimum, document it next to "No Cortex, no protection". Better, also treat a session as sandboxed when the system prompt's working directory has a .sbx/ binding. The script runs on the same machine, so it can check that.
The reason will be displayed to describe this comment to others. Learn more.
nit: Name the moca version or commit this was copied from (it matches CLAUDE_DENY at rossoctl/moca bbeca5f4, 17 tools), so drift can be spotted until the deferred CI comparison exists.
The reason will be displayed to describe this comment to others. Learn more.
suggestion:--no-agent-config also skips opencode.json, the CLAUDE.md/AGENTS.md rules block and the .sbx/agent-config.json record. As a result, mocactl sbx rm will not undo this claude mcp add, and mocactl sbx doctor reports its agent-config check as failed. Worth adding claude mcp remove -s local sandbox as a teardown step, plus a note that doctor's agent-config failure is expected under this setup.
The reason will be displayed to describe this comment to others. Learn more.
nit:rossoctl/moca#568 is closed. rossoctl/moca#572 moved moca's sandbox steering into the MCP server's instructions, so that part now reaches the model. What the deny still blocks is the project's own CLAUDE.md, which is the case this example fixes. Suggest rewording to say so.

Problem
mocactl sbx updenies Claude Code's local tools in the repo's.claude/settings.json, so the agent only uses the remote sandbox's MCP tools.Readdeny also stops Claude Code from loadingCLAUDE.md(sbx up's agent config: the CLAUDE.md steering never loads (Read deny), and stale deny names warn on every startup moca#568). A project's own instructions never reach the model. What this does
With Cortex in the path, a moca repo needs no

Three gaps in the script plugin showed up in end-to-end testing and are fixed here:
fail_closedslot refuses while a script did not start. Before, a script that failed to start was skipped and localBashran. With it after, a user's response-reading script made the config fail to load. The script is an example in

fail_closedenabled now reject requests with a 502 error when a script fails to start. In observing mode, the failure is recorded and requests continue.
